INDIVIDUAL REGISTRATION FORM FOR CLINIC ONLY

Deadline:	JUNE 5, 2015
Individual clinic rate:	$50.00 per GACA member who pre-registers.
Group rates:	$45.00 per coach for 5 or more GACA member coaches from same school who pre-register! Please use the Group Registration form for more than one coach from the same school $60.00 per coach at the door for all GACA members
Non-member rates:	$80.00 per coach for all non-members
These fees do not include GACA membership dues for the next school year 2015-2016!
Location:	James H. Rainwater Conference Center 1 Meeting Pl, Valdosta, GA 31601
Hotel:	Holiday Inn Valdosta Conference Center 1805 West Hill Avenue, Valdosta, GA 31601 $89.00/Night + free breakfast

THERE IS A DEADLINE OF JUNE 15, 2015 FOR THE REDUCED GACA RATE FOR A GUESTROOM. IF YOU DO NOT MAKE YOUR RESERVATION WITH THE HOTEL BEFORE JUNE 15, 2015, YOU WILL HAVE TO PAY THE REGULAR RATE!
